Provider & Pharmacy Directories

Wisconsin Personal Care Plan’s provider network includes doctors, clinics, hospitals, skilled nursing facilities, home health agencies, pharmacies, health care facilities and other health professionals. You will get most of your routine or basic care from your Primary Care Clinic, and your Primary Care Clinic will coordinate the rest of the covered services you get as a plan member.

Wisconsin Personal Care Plan may add or remove providers and pharmacies from our directories during the year. To get current information about network providers and pharmacies in your area, please call us at 1-715-552-4300 or 1-888-203-7770 (toll free). Our normal business hours are Monday through Friday, 8:00 am - 5:00 pm; a voice mail system is available for all after hour calls from 5:00 pm - 8:00 pm, and during weekends and holidays. The fact that a provider, clinic or hospital is listed in this directory does not automatically mean that the services are covered under your employer's plan.
