Formulary for Wisconsin Personal Care Plan

Wisconsin Personal Care Plan uses a formulary, which is a list of all the prescription drugs we cover. The drugs on this formulary are selected by our Plan with the help of a team of health care providers. Both brand-name drugs and generic drugs are included on the formulary. We may periodically add, remove, make changes to coverage limitations on certain drugs or change how much you pay for a drug. If we make any formulary change that limits your ability to fill your prescriptions, we will notify you before the change is made.
We will generally cover the drugs listed in our formulary as long as the drug is medically necessary, the prescription is filled at a network pharmacy, and for certain drugs other requirements are met. Some medications may not be on our formulary, or may require a prior authorization. For more information about formularies, please read the “Annual Notice of Change/Evidence of Coverage (ANOC/EOC)”.
