Wisconsin Personal Care Plan is a Medicare Advantage Special Needs plan, offered by Group Health Cooperative of Eau Claire. Wisconsin Personal Care Plan is a Health Maintenance Organization (HMO) that contracts with Medicare. This plan is for people who are full benefit dual eligible, which means they have both Medicaid and Medicare.

The Wisconsin Personal Care Plan service area includes the following six counties in West Central Wisconsin: Buffalo, Jackson, Trempealeau, La Crosse, Monroe and Vernon counties. You must live in one of these six counties in order to join the plan.
